Barrel Lug Broken on Duty

A firends new  P07 Duty jammed after about 30 rds and when he pulled the slide back with difficulty we found the barrel lug had broken off. Will send pictures and serial number in a day or two.
I am sure it is a rare and freak accident; we have thousands of CZ p07 Duty pistols in use in Pakistan but this is the only incident and surprises us all.
What could have caused it? The barrel does not show any signd of over pressure the case came out normal and clean  and there are no signs of any damage to the pistol at all.

I sure hope it is a rare incident.  I have about 2500 rds through mine now with nary a problem.  With the lug coming off like that, almost sounds like what you get in electronics with a cold solder joint, a brittle, non-functioning connection.
