CZ 712 trigger not resetting

I shoot my CZ 712 in multigun and 5 stage. I have been having an issue with a "dead" trigger occasionally. Mostly occurs when shooting 5 stage. It happens once or twice per match. I have cleaned and lubed my gun, as well as removed and cleaned and inspected the trigger assembly. I noticed the trigger wasn't reset the 2nd time I experienced the dead trigger at today's match. It has occurred with Federal 1200FPS and Challenger 1290 FPS ammo. It occurs while having 1 round in the chamber and then one in the tube. It always occurs on the 2nd shot, the round is fully chambered and only clears when fully racked. Any help or ideas would be appreciated.
